Before you schedule an appointment, please fill out the New Patient Inquiry Form.
This short form helps us understand your needs.

Please fill out these forms at least one day before your visit. This gives your clinician time to review your information.

• Patient Intake Paperwork
• Adult Intake Rating Scale (for ages 18 and older)
• Minor Intake Rating Scale (for ages 5 to 17)

In order for our office to coordinate with other clinicians, therapists, family members, etc. we would need you to complete the following form:’
